Referrals
Please direct all patient enquiries or referrals to our Medical Admin team in the General Office on 01325 254321.
We accept referrals from members of the Primary Health Care Team (in particular GP's & District Nurses), Macmillan |Nurses, Hospital Doctors & Nursing Staff, & Allied Health Care Professionals. Relatives, friends or patients themselves can also ring direct to discuss referral to all our services with a member of staff.
Referrals can be made for any of the following services:
- In-Patient Admission
- Hospice at Home
- Day Care
- Family Support (Social work and counselling)
- Bereavement Support (Patients, Carers, Children)
- Lymphoedema Clinic
- Symptom control
- End stage care
- Respite care
