Referrals
Please direct all patient enquiries or referrals to Patricia Hennessy or Lynne Wright (Medical Admin) on 01325 254321 .
We accept referrals from members of the Primary Health Care Team (in particular GP's & District Nurses), Macmillan Nurses, Hospital Doctors & nursing staff & Allied Health Care Professionals. Relatives, friends or patients themselves can also ring direct to discuss referral to all our services with a member of staff.
Referrals can be made for any of the following services:
- In- Patient Admission
- Hospice at Home
- Day Care
- Bereavement Counselling
- Lymphoedema Clinic
- Symptom Control
- End stage care
- Respite care
