Referrals can be made for any of our services, and we accept referrals from GP’s, District Nurses, Macmillan Nurses, Hospital Doctors & Nursing staff and any health care professionals.
How to refer
For patient enquiries or referrals, contact our Medical Admin Team on 01325 254 321 Monday-Friday 8.30am – 4.30pm.
Relatives, friends or patients themselves can also ring direct to discuss referral to all our services with a member of staff.
Referrals can be made for any of the following services:
- Inpatient admission (including planned respite care)
- Community Hospice
- Outpatient appointments (including Lymphoedema Clinic and Complementary Therapies)
- Family Support (social work and counselling)
- Bereavement Support (adults and children)
We accept referrals from members of the Primary Health Care Team (in particular GP’s, District Nurses), Macmillan Nurses, Hospital Doctors & Nursing staff and any health care professionals. Please complete a referral form by clicking the links below and return these to the Medical Admin Team.
Only people wishing for a referral to our Inpatient Unit need to complete the second page.
Responding to a referral
A member of the relevant team will contact you and discuss what happens next with you.
We may also liaise with other healthcare professionals to ensure we can provide support based on your individual needs.
How to get advice
For none urgent advice, contact our main Hospice number on 01325 254 321.
We will only give medication and prescribing advice directly to a prescriber/ health care professional.
Need urgent help?
If you have concerns and would like to speak to someone immediately outside of normal office hours (Monday-Friday 8.30am-4.30pm), please contact NHS 111. If you have been referred to the Rapid Response Team you can contact them 24 hours a day, 7 days a week. For more information please click here
